Khatauli (Uttar Pradesh), Aug 21 (ANI): The Commissioner of Railway Safety Shailesh Kumar Pathak admitted that the Utkal Express train derailment was a human failure. He further said the matter needs to be investigated as there are many loopholes in the information that is already being circulated in news. Pathak further assured that actions will be taken against the culprits.
